[ixpmanager] Patch Panels

Can we get the capability to delete ports within a patch panel? Currently when I mess something up, I have to delete the whole panel and start over. Fine now that IXP Manager isn't in production, but a bit more of an issue later.
Can we get the ability to edit port names? As we should be entering each port and my panel only has labels for the duplex port as a whole, I was going to append L and R to the end of each existing port to signify the left and right ports in the duplex port. I went looking in SQL, but I don't see where the port name is stored to just modify it there.

This includes adding a free text field in patch panels which is
explained in the above.

Also added a new section on duplex ports to the documentation.
The tl;dr version is: IXP Manager offers flexibility, you need to make a
decision on how best to use it to represent your own scenario.
