[ixpmanager] Adding Switch

First, delete the switch, and use the "Add New Switch (via SNMP) "
wizard to re-add the switch.

If you put the hostname (<x.y.z.w>) into the Hostname field, and
whatever you used for <snmpcommunity> in the "SNMP Community" field, it
should work fine.
Don't use the "Manual / Non-SNMP Add" option.
Also, as it's a cisco switch, please make sure that the SNMP community
does not contain the "@" symbol.
